Vaccination Verification

As per New York State, and Nassau County guidelines, ALL tournament participants (coaches, judges, students, observers) MUST be fully vaccinated. Please use the following Google Form link to upload images of the fronts of vaccination card for each participant. Each form is set to allow up to 10 images. If you have more than 10 participants, you'll need to fill out another form.
